There are several ways to order on-line registration form numbers
1. Ring the office during office hours and you can pay over the
phone with a credit card
2. Use the new order form and send it in with a cheque or your
credit card details
3. Fax in the order form with your credit card details
If you order by phone, fax or e-mail with a credit card we will try to
issue the forms immediately. We are planning to set up an on-line
shop so that you can purchase forms without intervention from the
office

name you may

minimum of three
letters or you will
