The Importance of the Young
Adult Worker
Royal Free Hospital
The Young Adult Worker role
• Trained Youth Worker
• 3 days a week, 2 years funded by BKPA, now full
time
• Social, emotional and practical support
• Sessions in hospital, home visits and/or informal
meetings
The clinics
Young adult nephrology clinic
• Once a month
• Adult and paediatric nephrologist
• Patients with nephrotic syndrome and other forms
of chronic kidney disease (CKD)
Renal transplant clinic
• Twice a week
• Part of adult transplant clinic

• Most referrals from GOSH/ South East England
• 30 min clinic slots for new patients
Renal transplant clinic
• Most referrals from GOSH – quarterly transition
clinic
• Informal visit at Royal Free

Key Positive Outcomes
• Smoother transition journey addressing young
people’s needs
• Improved communication between patients and
hospital
• Decreased DNA (did not attend) rates overall
• Support for young people in social issues as well
as health enabling them to take better care of
themselves
Patient Feedback
• Patients report feeling more confident when able to meet
their peers at social gatherings
‘I’ve got more independence and confidence being here. I was put out of my
comfort zone but that was good. I’ve learnt how to communicate with others’
