OHIO University Student Health Insurance Policy
• All students are required to have an active health insurance
policy while attending OHIO University
• Your health insurance is provided by a commercial company
named United Healthcare Student Resources.
• Your insurance is valid July 26, 2015 through August 15, 2015
INSURANCE ID CARDS
• Insurance Identification Cards are provided to you in
your insurance information packet
• You will need your identification card to:
– Receive Medical Treatment
– Purchase medications from a pharmacy at a reduced rate
WHAT DOES THE INSURANCE PAY FOR?
The health insurance benefits at Campus Care are for:
• An office visit to see a doctor if you are sick or injured
– You will owe a $15.00 copay to see a doctor at Campus Care
• Laboratory testing for diagnostic purposes
• X-Rays (Radiology)
• Reduced rate for medications from the Campus Care
pharmacy
– You will owe $5.00-$35.00 for medications purchased at the Campus Care
Pharmacy
WHAT DOES THE INSURANCE NOT PAY FOR?
The Health insurance will NOT pay for:
• An office visit to see a doctor when you are not sick, or do not
have an injury or illness
• The $15.00 copay to see a doctor at Campus Care
• Dental Treatment
• Vision Screenings
• Medical treatment for items listed on pages 3-4 of the policy

• Access claim letters
• Complete Accident Details form
