Did you administer the Horizontal Gaze Nystagmus Examination ?

The subject did/did not indicate the use of corrective lenses. The subject could/could not follow the stimulus and the eyes did/did not
track equally. The subject’s pupils were near normal/larger than normal/smaller than normal and both pupils were/were not the same
size. Based on these observations the subject did/did not participate with the horizontal gaze nystagmus examination. I first checked the
eyes for a lack of smooth pursuit by moving a stimulus left and right to check to see if nystagmus was present. I did/did not observe a lack
of smoother pursuit in the left eye. I did/did not observe a lack of smoother pursuit in the right eye. I checked for the lack of smooth
pursuit in each eye twice. I then checked for distinct and sustained nystagmus at maximum deviation. I did/did not observe distinct and
sustained nystagmus at maximum deviation in the left eye. I did/did not observe distinct and sustained nystagmus at maximum deviation
in the right eye. I checked for distinct and sustained nystagmus at maximum deviation in each eye twice. I then checked the eyes for onset
of nystagmus prior to a forty-five degree angle. I did/did not observe an onset of nystagmus prior to or at a forty-five degree angle in the
left eye. I did/did not observe an angle of onset of nystagmus prior to a forty-five degree angle in the right eye. I then checked the eyes
for vertical gaze nystagmus twice by moving the stimulus upwards in a vertical direction and holding for a minimum of four seconds.
Vertical gaze nystagmus was/was not present. The total numbers of clues observed during the horizontal gaze nystagmus test were 5
