What is a Health Savings Account?
A Health Savings Account, or HSA, is a tax-free account that…
• Helps you budget for health care expenses this year.
• Allows you to build savings for future expenses.
• Is available only if you are enrolled in a High Deductible Health Plan.
To be eligible for an HSA, you must meet the following criteria:
• Have a qualified consumer directed health plan
• Have no other medical coverage except what is permitted by IRS
• Not be enrolled in Medicare Part A or B (spouse can be enrolled in
Medicare)
• Not be claimed as a dependent on someone else’s tax return (other than
as a spouse)
• Not have access to dollars in a Flexible Spending Account, including
spouse’s FSA

Affordable Care Act Update
2016 Tax Filing!
• Benefit eligible UC employees will receive IRS Form 1095-C
• Provides IRS with information about coverage status (enrolled or
not), covered dependents and period of time covered (if applicable)
• You will use the information on the form to complete LINE 61 OF
your Federal return
• You may need to provide copies to others (e.g., spouse) depending
upon filing status
• Paper form delivered to UC employees at home address by same
deadline as W2 (IRS postmark requirement is 01/31/2017)
• Will NOT be mailed from UC
Unum Voluntary Benefit Plans
Critical Illness/Cancer coverage pays a cash benefit if diagnosed with
serious illness/condition (i.e. heart attack, stroke, coronary artery disease,
cancer.
Accident plan provides off-the-job coverage for accidental injuries, hospital
care, accidental death benefits
Wellness benefit available with both plans
