Why are we talking about
Patient–Centered Medical Home ?
The current health care system can be confusing. You may visit
more than one doctor and feel lost in the system. Sometimes, you
may even have extra medical tests or appointments because of
seeing different doctors. The medical home team will efficiently take
care of you, listen to you, help you keep track of your health
information, and help you find your way through the system
