permanent record of each mandated vaccination
• Date of vaccination
• Route and site of vaccination
• Vaccine type, manufacturer, lot number, expiration date
• Name, address, title of person administering the vaccine
Reason for record
• To ensure appropriate vaccination
• To avoid overvaccination
