 Our Nurse Navigator Program is designed to support
and guide patients and their families on the journey
through cancer diagnosis, treatment, recovery and
survivorship

We work diligently to remove any barriers that stand between
the patient and effective comprehensive cancer care

 Meets newly diagnosed cancer patients and
provides educational materials
 Provides patient and family support
 Makes referrals to appropriate community
services such as SOS (Survivors Offering
Support), The Red Devils, Cancer Support
Foundation

 Multi-Disciplinary Conferences
 We arrange weekly disease specific conferences to ensure care
is coordinated on a prospective basis
 Develop a care plan for each patient
 We record conference committee recommendations used to
develop a treatment plan for each individual patient.

 Following conference, the patient and family will
return to meet with the doctor to review the
conference committee recommendations
 We educate and answer questions that the patients
have regarding the specific treatment care plan
decided upon

We ensure that patients follow up to appointments,
consultations and treatments

 The treatment plan is completely and thoroughly
personalized for each patient.
It of utmost importance to acknowledge the fact that
breast cancer is not just a physical diagnosis, but touches
the patient in all facets of his or her life.
 We support our patients and their families through
continuous interactions w/ our multi-disciplinary team as
well as outside resources.


