PARTICIPATION
• HANDLEY EMERGENCY CARD
• HANDLEY DRUG / ALCOHOL FORM
• VHSL PHYSICAL FORM
• INSURANCE WAIVER
EMERGENCY FORM
• The emergency card form:
• Allows the Handley athletic staff to seek
treatment for an injured student-athlete.
• Gives proper information on emergency
family contacts.
• Gives a medical history and background
on each individual student-athlete.
DRUG AND ALCOHOL FORM
• The drug and alcohol form:
• Gives parents and the student-athlete
information concerning the Handley
Athletic policies in dealing with drug and
alcohol use during a particular sports
season.
• Allows the student and parent to sign an
acknowledgement of the rules governing
drug and alcohol use of a student-athlete.
VHSL PHYSICAL FORM
• The VHSL physical form:
• Gives the proper information in regards to
the current physical well-being of an
athlete.
• Determines any physical limitations an
athlete may have.
• Gives pertinent information of the
background and history of the physical
condition of an athlete.
INSURANCE WAIVER
• The insurance waiver:
• Gives the proper information in regards to
insurance coverage a student-athlete and
their parents have.
