• Complete the five educational modules quizzes with a
score of 80% or better
• Print a copy of your quiz results
• Read information provided about Methodist Medical
Center
• Complete the HIPAA 101 training module
• Print, read and sign the Confidentiality statement
• Print and sign the HIPAA Training Documentation

• Print and complete the Immunization Record
• Give quiz report, confidentiality statement and
immunization record to your instructor.

Parking
Students are instructed to park in
employee parking located behind the
hospital off of West Tennessee
Avenue.
Smoking Policy
Methodist Medical Center is a non-smoking
facility.
Visitors/staff can smoke ONLY in designated
areas OUTSIDE the hospital.
An enclosed smoking gazebo is located
across from the Emergency Department.
