What you can expect
as a patient.
Caring for a vulnerable adult with a learning disability

The doctors and nurses will ask you about
what help you need.
We will use your ‘hospital passport’ which
tells us about what help you need. If you
don’t have one we can help you get one.
A butterfly symbol will be placed above
your bed, and a blue name-band on your
wrist. This will tell others that you may
need more time to talk about any help you
might need.
If you are having difficulty with
communication we have ‘Easy Read’
leaflets to help you understand your
treatment. Please ask a nurse for them.

‘MY NEEDS’
We will talk to you about ‘reasonable
adjustments’ - these are the things we can
do to make your stay in hospital as
comfortable and stress free as possible.
We will ask you about any special diet or
equipment that you may need. If you have
any worries about this please tell the nurse.
There will be people to talk to if you have
any concerns about your care. You can talk
to the Ward Manager or to the
Safeguarding Team (Tel 0161 206 1361 and
ask for Annmarie Hornsby or Sally Hulmes)
When you are well enough to go home
your discharge will be planned with you
and your carers.
You will take home your medications and a
letter called a ‘Discharge Summary’
explaining what treatment you have
received. A nurse will talk to you about these
before you leave. Your Doctor will also
receive a copy of the Discharge Summary.
