Faith Folder: Discharge Planning…and BEYOND
Faith Community Hospital’s discharge
planning program is an innovative
approach that includes patient teaching
on prevention and health screening. The
Faith Folder is based upon Project RED
(ReEngineered Discharge) and includes
teaching on the patient’s primary
diagnosis and medications as well as
arranging assistance with access to
appointments, transportation and
medication after discharge.
Patients are often focused more on
getting home and than what it will
take to manage care at home. The
Discharge Advocate may be
contacted after discharge with
questions or to receive clarification
by calling the hospital or by
sending an email to
[email protected].
The Faith Folder has several unique
features. It uses a proactive approach to
health care by providing more in-depth
teaching on prescribed diets, treatment
plans, and recommended health
screenings. The Faith Folder is an age-,
gender- and ethnic-specific resource for
managing care at home. Teaching is
geared toward the parents of children,
patients, or other caregivers depending
on the age and cognitive abilities of the
patient. When needed, teaching is
translated into the patient’s primary
language.
