So How Can You Integrate
Patient BP Tools?
 Download video files on laptops or tablets to share
with patients during visits when appropriate
– If paper-based HHA, make DVD with both videos and
provide one to each staff member
 Check with your insurers if they cover electronic
blood pressure cuffs
– FYI the Blue Cross Blue Shield Association for the Federal
Employee Program now offers free monitors
 Collaborate with local pharmacy or store for
discounted electronic blood pressure cuffs
– Create cute cards for patients to ask family members to
buy a monitor for their birthday or a holiday
What About the Workbook?
 Consider doing a pilot with the workbook
– Educate staff on the components of the workbook by
watching the videos or doing HHQI University courses
– Discuss how and where to document
– Roll out to one or two clinicians, get feedback, make
changes, try a few more clinicians or few more
patients, get some patient feedback… then SPREAD
 Can alter the workbook with your own tools and
definitely add your logo and contact information
 Restock the staff at team meetings
