TO THE
WHANGAREI HOSPITAL COPD PATHWAY AND
DISCHARGE BUNDLE
1. Standardize hospital management of
COPD within ED and on wards
2. Establish a bundle of care that is
reliably delivered to COPD patients
3. Patients are given a Blue Card with
clear steps of what to do when unwell
4. Patient is discharged with a prescription
for antibiotics +/- prednisone to be filled
and have available at home for an
exacerbation
5. One-off Community Pharmacy visit
(fully funded)
6. St John Ambulance to use a Risk
Stratification Pathway that encourages
patient to self manage exacerbations in
conjunction with GP
BLUE INFORMATION FOLDER
How can Primary Care assist in this project?
Consider using Blue card initiative coupled with action plan medication
prescription, so patient always has acute medications at home
Encourage COPD patients and close family to have Flu Vaccination
Facilitate COPD patients into huffers and puffers low intensity exercise
class. (Need GP sign off to attend on form )
Future directions – spirometry competency for training available to
practice nurses and ongoing linking with St John ambulance
