Nurse drew blood from the correct patient and then
postponed labeling the vial, which then resulted in a
later incorrect labeling.

and verify the accuracy of the blood type and the
patient.
Then two clinicians have to verify that the correct
blood type is being administered.
JCAHO suggests some help to reduce risk of error

It is hard to argue with data.
It is essential that people have the ability to accept
objective information and not hide from the facts.
Remember that people can accept financial failures
more easily than clinical errors.
