– Convert an e-form in any format to PDF
with Adobe Acrobat Professional
This
demo uses the common W4
form as an example
Create Database Tables
Create
database tables in any major
database such as Oracle, MySql or
MS Sql Server. We need
– One table for each form
– One table for each sub-form or grid
Create Form Template
Open GoFormOnline Form
Designer, go to the menu and
select Template|Create
Template
Enter the following
information and then click
“OK” to create a new
template:

Template settings can be
edited by selecting Menu
Template followed by Edit

Edit Form Template
Add Input Fields
Set default values

Set business rules
Set workflow. Sections 1 to 7 in
the W4 form are for end-users,
and 8 to 10 are for Admin to
process
Click the “Save” button. The
form is ready to be processed on
the web
You can add or remove any Input Fields
or change any settings. After clicking
the “Save” button, the changes become
effective on the web immediately.
Login to GoFormOnline
This system requires a login so that
– The form can be pre-filled with default information of the current user
– The form can be saved, submitted and processed by different users
who log on
– For this demo, we will use a test account with User Name: Jack, and
Password: test
– You can also create your own account to login

This page contains all forms and form packages
the current user has permission to fill
My Forms
Selecting a form, such as the W4, takes you to this page in the
Public User Mode
On this page, you may select to
– View a submitted form by clicking “View”
– Clone a submitted form by clicking “Clone”
– Open a new form by selecting “New Form”
You can switch to a different form using the dropdown list
Fill and Submit a New W4 Form
A new form is pre-filled
with personal information
The editable fields are in
light blue
The form can be saved to
the database on the server
by clicking the “Save”
button
The value for Section 5 can
be calculated by entering
values in A to G
If any input is incorrect, for
example, the social
security number not being
nine digits, an error
message will be displayed
Continue to next slide
If
the form is not completed and the
“Submit” button is clicked, GoFormonline
displays a message for incomplete form
and sets focus to the first incomplete
field
When
a form is submitted, a notification
email is sent to the next level
Administrator
After
a form is submitted, it becomes
read-only unless the form is rejected
later by an admin user
The form can be
printed at any stage
by clicking the
printer icon (“Create
PDF for Printing),
and then the open
folder icon (“Open
PDF for Printing”)
A Filled W4 form is
sent back to a new
browser window with
the Adobe Add-on
loaded for printing
Office Users
If the form is set to be filled by office users, when the user selects
a form such as W4, instead of going to “My Forms”, it goes to “My
Group Forms”
From here, the user can open a new form, or search an existing
form to edit or view by any pre-set fields
Fill Fields with Default Values in Office User Mode
In the Office User Mode, the new form is not pre-filled with user
information
When a field such as “Last Name” is clicked, a dialog box is
displayed and searches for a name by any pre-set fields
Click “Select” for a name in the search results, and all the fields in
the form will be filled with the information of the selected person
Admin Users
When a form (or a form package) is submitted, the next
level admin users receive notification
An admin user who logs into the system will see the links
“Process Forms”, “View Forms”, “Process Packages” and
“View Packages” at the upper-right part of the home page if
there are forms and packages to be processed or viewed
To process a form, select “Process Forms”, and choose the
appropriate form
To view a form, select “View Forms”, and choose the
appropriate form
A non-admin user with view permission can also search and
view a form
Process Submitted Forms
The form submitted by Jack
has three fields, 8 to 10, to be
edited by the admin. The
three fields are pre-filled with
default values for the admin.
The admin user can reject or
approve the form
If rejected, the admin should
explain reasons through an
email. The rejected form can
be edited and re-submitted
If the form is approved, it
becomes read-only
The workflow is not always
this simple. It often has
multiple levels and sub-levels.
Some sub-levels can be
processed concurrently
Assist Form Filling
A part of the form or the whole form can be assigned to another
person to work on. For example, the Patent Application Form has a
section for the lawyer
After a user opens and saves a form, the user can assign a role to
another person by entering an email
Choose the role for the assigned person and then click “Save”
The system sends an email to the assigned person
When the assigned
user logs in, a link
”Help Requested” is
displayed
Click the link and
select the form for
which help is
requested
On the third page of
the Patent
Application Form,
there is a section for
the lawyer to fill in
If a third party was
given permission to
submit, he/she can
complete the form
and submit it
