Process for Referrals
• Surgical and medical cases referral form
completed with instructions from regular
veterinarian or surgeon
– Referring veterinarian can request specific
activities, or
– Ask us to develop a rehab plan
Recreational Swimming
• Not a veterinary activity
• “Fit to swim” form signed by regular
veterinarian
• Or a signed waiver/release form
