WARD RETURNS OR BED STATE
In-patient data indicates the activities of inpatient services in
a health institution.
Data collected through a form commonly known as a Daily
ward bed return or Bed state
These are forms used in the wards.
It used to record inpatient data for 24 hours, start from 12.00
pm to 12.00 pm the following midnight.
It report the remaining number of patients in the ward.
Data required in the bed state form
Admission data with their registration numbers patient
names and both sexes together Number of discharges live and
death
Inter-wards transfers of patient.
Means that transfers in or transfer out.
Total number of patient who are remaining in the ward.
Its supposed to filled by nurse on duty.
Number of patient day in the ward
The counting number of patient yesterday, then
add (+) all incoming (admission and transfers in)
and subtract (- ) all outgoing patients (discharges
live, transfer out, abscondes and deaths)
The new total is the number of patient today and
will be brought forward till next day census

The purpose of bed state
 Preparing report for the patient who admitted at the
hospital daily, monthly and years.
 Many hospital use bed state as the source of information
for input process and out put of the hospital demand.
 To know number of patient admitted in each ward.
 Determination of number of male and female admitted at
the hospital.
Bed state form
 The filled bed state forms for all wards should be
submitted to the health records department where
summary for whole hospital records is made up.
