1. Anticoagulation chart





It is a prescription and monitoring chart
It has instructions and space for prescription
and monitoring of un-fractioned heparin
infusion
It has information to make you think about
other antithrombotic agents including the
novel anticoagulants
It is a referral form for anticoagulant clinic
It has 6 pages

2. Insulin S/C






It is a prescribing, monitoring and administrating chart
It gives an opportunity for patients to self administer if they wish
to do so after being assessed
It has a glucose monitoring section that is designed to alert
prescribers/ administrators if the patient blood glucose is not
stable
You can now document site of insulin injection at each dose
There is information about how to treat hyopglycaemic patient






Designed for NIL by mouth patients on insulin whom

It has a section for intravenous fluid to be running
along the insulin infusion
