BACKGROUND
• Burns Lake Hospital is a 13-bed acute care
rural hospital
– With a full-time “stand-alone” Pharmacy Technician
• One of Northern Health Authorities’ Medication
Reconciliation test sites
AIMS
• To prevent Adverse Drug Events (ADE’s)
by implementing medication reconciliation at
admission in Burns Lake Hospital
• Goals:
– To increase the effectiveness of the medication admission
reconciliation process over time
– To reduce the rate of Unintentional and Undocumented
Intentional discrepancies at admission by 75% in 6 months

• 4 hour Med Reconciliation training workshop
• Pre-Admission Medication Verification Tool
• Best Possible Medication History Checklist
– With verification & discrepancy codes
• Laminated pocket card BPMH support tool
– OTC medications - Review of systems
– Social history guidelines
