An overview…
 The team consists of 3 Outreach workers and was
launched in November 2010
 Objective: to increase throughput and positive exits
– Re-engagement of potential ‘unplanned exits’
– Targeted ‘Recovery Reviews’
– Residential Rehab preparation group
– Aftercare provision
 The team work concurrently with the existing care
coordinators

 All 3 have excellent IT skills which have allowed the
increased use and interrogation of client data
 Each cover a specific locality and have worked there
for a number of years
 Attend weekly team meetings in local CDT’s
 Weekly Peer Supervision, monthly PSI supervision
Re-engagement (that sticks...)
Aims…
 to re-engage back into meaningful treatment
 exit drug free / occasional user
Through…
 Telephone, E-mail, text message, home visits etc…
Therefore…
 Reducing unplanned exits & increasing service user
ownership of their treatment journey
Of the 36 clients re-engaged in the last 6months, only 1 has
dropped out of treatment since
