NEW REGISTRATIONS
We are currently accepting new patients.
If you would like to register with the Practice, you will
need to bring two forms of identification:
1. Your passport or driving licence – to confirm
your identity.
2. A utility bill – to confirm your address.
Please ask at the reception desk for further
information.

DIY Health Check
Would you like to know your blood pressure?
Would you like to know your height and weight?
Please feel free to use our blood pressure
monitoring machine, electronic scales and height
measure located in the room to the left of the
reception desk.
Please complete the form provided (in the room) and
hand it to someone on reception.
PATIENT INVOLVEMENT GROUP
Shipley and Westcliffe Medical Practices now have a
joint Patient Involvement Group.
If you would like find out more about the practices and
have your say on future services come along to the
next meeting:
Thursday, 5th May 2016 - 4.30pm

Our practice nurses can give you all the help you need
to free yourself from the addiction with medications,
positive advice and support.
RESULTS
Please remember that you must phone or call in to the
surgery to receive test results.
Some results take longer than other and therefore we
advise that you wait 7 days for results.
Please bare in mind that our receptionists are unable to
and are not qualified to answer any questions you may
have regarding your test results.
SELF ARRIVAL SCREEN
You may have noticed our Self-Arrival Screen on the
wall to the left of reception.
It is quite simple to use, just touch the screen and it
will ask you for:
• The date and month you were born
• The first letter of your surname.
COMPLAINTS
If you have a complaint or concern about the service
that you have received or any member of staff please
let us know.
If you would like further information on the complaints
procedure please ask at reception.
Likewise if you would like share any positive feedback
in relation to the practice or individual member of
staff, please do so.

SEXUAL HEALTH CLINIC
We are no longer running a sexual health drop-in
clinic on Thursday evenings.
Locala Bradford Sexual Health is a
free and confidential Contraception
& Sexual Health service across
Bradford & Airedale.
