Stroke/Transient Ischemic Attack
Brain Anatomy/ Physiology related to stroke event: Document any basic
neuroanatomy/physiology concepts that you discussed with the patient/family/friends.
Stroke pathophysiology and potential causes: When you talk to the patient/family/friends
about what caused this stroke.
Stroke Recovery Process: When you talk to the patient/family/friends about therapy for
mobility, communication, strength, rehab needs and psychological counseling.
Rehabilitation Techniques for Self Care/Functional Independence: When you teach about
aspiration precautions, repositioning, splint usage, providing rest periods for the patient.
Risk Factors for secondary stroke and plan to decrease risk factors: When you teach about
the risk factors or secondary complications specific to this stroke (i.e. dysphagia, DVT,
diabetes, hypertension, smoking).
Potential physiological problems/ signs & symptoms, prevention, follow–up: When you teach
about the 5 items in the discharge summary (i.e. risk factors, warning signs, prescribed
medications, need for follow-up after discharge, calling 911).
Lifestyle Alterations, present and future: When you teach about support groups, coping skills.
