Who We Are
An interdepartmental group of hospital employees.
Long, Medium and Short Term Outcomes
• LONG TERM: To improve proper hand hygiene
adherence to 90% or above at our facility.
• MEDIUM TERM: To reduce our adherence
deficiency by half before the end of 2015.
• SHORT TERM: Educate and enlist staff in the
importance of proper hand hygiene in the
healthcare setting.

Eliminating HAB A Change for the Better
North Sunflower Medical Center, a critical access
hospital in the heart of the Mississippi Delta, is
located in Ruleville, MS.
“Take me to Ruleville.”
North Sunflower Medical Center exists to improve
the lives of people in the Delta. We work together
with the community as a family.
Our Safety Team is comprised of:

Pearls
• We found that adding a Discharge planner has helped our
patients get a better understanding of their medications and
expectations for their return home.
• Implementing a Good Catch program has helped identify
adverse drug events before they get to the patient.
• Starting a Foley log has helped track the correct amount of
insertions and catheter days.
• We recently started a handwashing campaign and are
anxious to see the results.
Pearls
• Reviewing all falls to drill down to causes, times,
any other pertinent data
• Established a fall team to review data and make
recommendations
• Reporting on falls and goals to the hospital board.
