MMS Practicalities
 Accredited MUR pharmacist(s)
 Private consulting room
 Aim 1 - 2/week
 Regular time slot - calendar
 MMS takes 3/4 hr @ pharmacy or 1 hr
home visit ( plus travel)
 Allow ½- 1 hr for prep/write up

Referral Process
 Dispensary staff flag prescriptions e.g.




multiple medications/ prescribers
recent discharge from hospital
new medications or dose changes
LTC patients
 Pharmacist discusses MMS eligibility
with patient / family member
 Give brochure /consent form
 Record patient reply in computer
Other Referrers
 Patient self-referral
 GP/ Practice nurse
 Secondary Care, e.g.
• hospital pharmacist, outreach nurse
 Primary Care, e.g.
• district nurses/nursing providers/allied health
 Family/ Whanau
Before MMS review
 GP notification letter faxed
 Prepare MMS documents (Checklist)
Print patient Hx - dispensing computer
Fill in MMS template
Commence MMS database entry
Patient telephoned on scheduled day
During MMS Review
 MMS in consulting room or patient’s home
 Consent form signed
 Document patient info e.g demographics
 Motivational Interviewing
 Assess medication understanding/perception
Discuss medication taking behaviour
Focus on adherence issues/barriers
Prioritise concerns with patient
Agree patient plan
After MMS Review
 Details transferred to MMS database
 Yellow medication card/patient plan
 Print outcome report – send for payment
 GP report faxed to GP and referrer
 Update details in dispensing computer
 Store documents in MMS folder (monthly)
 Fill summary sheet (follow ups,payments)
