Facts:
Introduction to
Services
Isabella County is an area that has a
very clear need for affordable
health care services as well as
wellness education.
This group plans to develop a clinic
to provide services to the less
fortunate. This population includes
the uninsured and under-insured
citizens of the community as well
as the large population of college
students in the area.
The services will be provided for
free or low cost, with a goal of
improving the overall health of the
citizens of Isabella County.
• In 2009, Isabella county had an
unemployment rate of 17%, with the
national average at 9.4% (City-data, 2010)
• Isabella county has a higher than state
average of binge drinking and motor
vehicle crash death rate (Snapshot, 2010)
• Isabella County is home to Central Michigan
University, adding to the population of
uninsured citizens.

Provide inexpensive services to the
poverty stricken, underserved, and
uninsured individuals within Isabella
County.
Assess and treat acute health problems
that are non-critical in nature.
Mobilize volunteers to serve as staff and
educators at the clinic.
Provide education to both healthy and
chronically ill patients in order to
promote their overall health.
Address the area’s obese population and
their health needs.
Decrease the County’s MVA fatalities
through community education.
Decrease sexually transmitted infection
(STI) instances within the Isabella
County community.
How do we get there?

Assess and Treat
Offer Classes
The clinic will provide treatment to
walk-in patients.
We will provide nutrition classes on a
bi-weekly basis.
We will screen for health problems.
The clinic will hold bi-weekly diabetic
education sessions.
We will gain access to low or no-cost
medications for needy patients.
We will evaluate all incoming clients’
BMI value and offer information
on healthy living.
The clinic will provide routine exams
such as pelvic exams and
pregnancy testing.
We will provide smoking cessation
support.
We will talk to high school students of
the risks of alcohol consumption.
We will work with the area college to
support pregnancy and STI
prevention.
Access available funding to offset the
cost of caring for patients that
cannot afford their care.
Obtain free medications and coupons
for discounts for patients that
cannot afford their medications.
Use free and low-cost staff to assess
and treat walk-in patients.
Recruit volunteers to provide health
care needs and education services.

REGISTERED NURSE EDUCATOR
General Summary of the Function/Purpose of this Position:
Provides on-going evidence based training programs to staff
and volunteers. Responsible for maintaining the training files
and files associated with licensing and accreditation.
Duty 1: Provides evidence based training programs in general
and specific nursing care, dietary training, mental health, and
client relations and client care. Delivers nursing focused
educational programs to clients. Provides health care
instruction and education to individuals relating to the
promotion of health and prevention or management of illness,
disease, injury, or disability. (60%)
Duty 2: Evaluates staff and volunteer competencies on an annual
or as needed basis. Maintains training and accreditation files
ensuring that the clinic is in compliance with all standards
and legal requirements. (30%)
Duty 3: Evaluates printed and electronic resources that are
available and directed at client health maintenance and care.
Maintains a current supply of free and low-cost materials and
supplies available for client wellness. Maintains an accurate
list of other agencies and resources available to clients in the
community. (10%)
Education Required:
Possession of a Bachelor’s, or Master’s degree in nursing
accepted for licensure by the Michigan Board of Nursing.
Experience Required:
One year of professional experience providing nursing care
and services.

NUTRITIONIST
General Summary of the Function/Purpose of this Position:
Instruct patients, families, and health care staff in
individualized diet therapy, general nutrition principles,
and food selection and preparation. Conduct nutritional
screenings and assist in the development, review and
modification of individual’s therapeutic diets.
Duty 1: Serves as the team’s expert and acts as nutrition resource
person on the health care treatment team. Develops,
reviews, and adapts therapeutic and modified diets to
meet the needs of individual residents and/or clients in
cooperation with health care staff. Inspects food
preparation and food service for conformance with
prescribed diets and standards of sanitation, palatability,
appearance, and nutritional quality. Plans menus for
clients at the agency and adjusts menu cycles for
nutritional adequacy, balance and variety, food
availability, and budgetary control. (60%)
Duty 2: Calculates nutritional values of food served. Determines
serving size, frequency, and method of preparation of
foods. Inspects food supplies for quantity, quality, and
compliance with state and federal standards.
Recommends type and quantities of food to be purchased.
(30%)
Duty 3: Recommends purchases of food preparation equipment.
Maintains records and prepares reports and
correspondence related to the work. (10%)
Education Required:
Possession of a bachelor’s degree in foods and nutrition,
or institution management.
Experience Required:
One year of professional experience providing dietetic and
nutritional services

Stayin’ Alive will provide a community in need with
much needed health services.
We have a plan to develop this service and to obtain
and utilize resources in a responsible manner, in
order to maximize the benefits to the community.
We will develop a clinic that will not only treat those
in need, but provide ongoing educational services
to improve the overall health of the community.
