LESSONS LEARNED
• Original Plan – Rely on MedsCheck to
get ‘Preliminary BPMH’
• Barrier – Most patients move into
Mount Hope within 72 hours of being
offered a room so getting a MedsCheck
done is not as realistic as we had hoped

NEXT STEPS
• Social workers to pilot an admission script
• Pilot the BPMH/AMO tool on one floor where
all the patients have a common physician
• Train the pharmacists and pharmacy
technicians how to use the electronic medical
records system to document completion of
MedRec on Admission and any discrepancies
