An electronic health record (EHR) is a secure and private lifetime
record of an individual’s health and care history, available
electronically to authorized health providers. It facilitates the sharing
of data – across the continuum of care, across healthcare delivery
organizations and across geographies.

patient. By definition it is not
complete, although it may be
more richly detailed in some
aspects than an EHR. It is
maintained by providers to
meet their fiduciary duties to
their patients, as well as the
requirements of their regulatory
bodies.
