Sliding Fee Program
• A sliding fee program for incomeeligible patients ensures that no one
who needs care is turned away
because of a lack of insurance or
other financial obstacles
• In 2008, 93% of PCHS uninsured
patients participated in the sliding
fee program
• That represents nearly 25% of our
total patient population

Sliding Fee Program
How it works:
• Patient brings income verification to first clinic visit.
- Examples of income verification are: current pay stub; income
tax forms; unemployment, social security or disability
voucher; letter of support
• Patient is required to pay a minimum of $20 per visit
• Patient meets with financial specialist to complete
application for income-based fee for visit
• Often the visit is fully covered after the $20 payment
• Patient can set up payment plan for as little as $5/month
