“Medication Reconciliation” is one of the
efforts to reduce the number of medication
errors which occur world-wide every day.
It is important that every person be
responsible for their own health care and
what happens to them.
This is a way to help all of us be more health
conscious.
WHERE DID THIS COME FROM



There are eight (8) national patient safety
goals.
All medical care entities incorporate the
national patient safety goals into their facility
quality of care programs.
The 8th goal reads: “Accurately and
completely reconcile medications across the
continuum of care.”
8th NATIONAL PATIENT SAFETY GOAL

This goal has been interpreted to mean:

Review and document with the patient a complete
list of each patient’s current medications upon
admission to the organization (clinic, ER, or care
facility, i.e., hospital or nursing home).
This information shall be communicated to the
next provider of service upon referral or transfer
within or outside the organization.
