This report makes projections
about how the student is
expected to perform on
the PLAN test in 9th grade,
the iACT test in 10th grade, and
the ACT test in 11th grade.

Your projected scores
College Readiness
Benchmarks
It also compares your
projections to ACT’s College
Readiness Benchmarks. Even if
you do not plan to attend
college, it is useful to see how
your projected scores compare
to the benchmarks.
Students scoring at or above the benchmark in each
subject area are projected to be on pace to succeed in
high school and college classes, if they keep up with
their coursework.
Students scoring below the benchmark will need to
put in extra effort and seek out additional supports to
improve their scores for future tests.

 Middle school teachers will use these projections to
recommend you for the appropriate level classes in English,
Math, History, and Biology.
 Middle school teachers also take into consideration other
factors, including:
 Current grades and classes
 Previous performance in class
 Interventions and progress monitoring
 Reading and writing ability
 Work ethic and motivation
 You’ll have the chance to review these recommendations in

school, and the effort you
put into your studies, you
may do better or worse
than projected.
You will be able to request
a different level class after
you receive your
recommendations in
January; the form is
available on the website.

New Freshman for fall of 2015……
 **ALL Freshman students are REQUIRED to have a
PHYSICAL completed and dated on or after AUGUST 1st
2014 or later on the 2 sided Illinois State form
 IHSA SPORTS PHYSICALS DO NOT FULFILL this
REQUIREMENT
 **All immunizations must be up to date including TWO
(2) doses of Varicella vaccine (Chickenpox)
 ** PARENT PORTION ON TOP OF SECOND PAGE must
be completed and SIGNED by a parent
 ++ALL MUST BE COMPLETED IN ORDER TO ATTEND
SCHOOL++
 Summer Reading is essential
 Must have physical AND TDAP
vaccination on the first day of
school
 PE Dress and Participation is
mandatory to get credit
 No social promotion-must earn

 Sophomore status is attained

Earning a passing grade in a course
results in .5 credit toward the
required 26 for graduation.
Students typically earn 3.5 credits
per semester, which totals 7 credits
per year.
A transcript is a permanent
academic record that starts with the
first semester, freshman year.
Transcripts are required by colleges,
the military, employers, scholarship
committees, and more.
Students must pass required
courses, repeating if necessary.
We have a club or sport for
everyone!
Visit the field house to obtain
information, meet sponsors
and students and ask
questions!
Colleges love to see students
involved in extracurriculars-it
shows that students are wellrounded.
Being involved in
extracurricular activities helps
students make friends, feel
connected to the school, and
enhances their classroom
learning.

NEXT STEPS for current D300 middle school

Before January 7:
Make elective requests (online preferred, via the Student
Portal).
Week of January 12:
High school counselors will visit D300 middle schools and
feeder parochial schools/charter school during the day to
review elective requests with each student.
Week of January 26:
Students will receive a copy of their core course
Nextrequests.

Week of January 26:
If you have questions about core recommendations,
contact the appropriate high school division head or
administrator. Submit level change requests to division
heads at this time.
Please note: Students will not receive a copy of

Pick up the registration packet tonight

Contact the School Registrar, Martha Silva-Zavala to
provide completed registration paperwork AND proper
documentation of residency
January
After the student in registered, Martha will schedule you
with a Jacobs School Counselor to discuss course selection
January
Work with Jaime Van Horn to get core placement
recommendations from middle school or parochial school
