 18
year-old black male , from
Rio de Janeiro , Brazil
 Blurred vision OS since 1 week
 History of being scratched by
domestic cat several times
July 2013 - First Presentation




VA : 20/20 OD / Counting fingers 2 M OS
no cells in the anterior chamber
IOP : 15 /16 mmHg
Fundoscopy: disc edema, macular edema,
retinal infiltrate in the inferior arcade
Diagnosis and Treatment
The diagnosis of neuroretinitis was made
 Cat-scratch was considered
 Patient was started on Doxyciclin 2x100mg
 Bartonella henselae serology was ordered
 Bartonella henselae IgG + 1/1024
 Treatment for 1 month
 Last VA (8/2013) 20/40

Conclusion




Cat-scratch disease may presente with very poor
visual acuity
Cat-scratch disease shows good response to
therapy with antibiotics
History is important in the diagnosis
Retinal infiltrates are very common in Cat-scratch
disease and may help in the diagnosis
