Directions
• Administer 1 on 1
• Each assessment should be completed at
the end of the month stated
• Provide manipulatives as needed
• Use checklist to monitor student progress
Question 1: Number Sense
• Start at 67 and count on
– Stop the student after 5 numbers
Question 2: Number Sense
• Count by 10’s to 100
Questions 3-4: Number Sense
• Provide students with counters or any available
manipulative
• Tell student to write the number 0
• If students write incorrect number, correct him or her before
continuing to the next question
• Tell the student - “Show me 0 counters.”
• Repeat with the numbers19
Note: If student writes a single digit backwards, it is still counted as correct.
Question 5-6: Number Sense
• Show the number 13
• Ask students:
– What is the number that comes before 13?
– What is the number that comes after 13?
• Repeat for the number 76
Question 7: Number Sense
• Using flashcards, have students order the
numbers 19, 20, and 21 from least to
greatest
Question 8: Number Sense
• Give students 23 counters
• Have them make groups of ten
• Ask students “How many groups of ten?”

Allow student the time to answer, however…
Students should answer within two seconds (1 mississippi,
2 mississippi) to be considered fluent and earn points.
Score is based on number of facts answered fluently (out of
five).
