Patients referred from Unit to Centre
• 11% referred using a proforma
• 44% referred via the telephone
• 55% presented at MDM
• 38% referred via other methods including
letters, faxes and electronic instructions

Treatment Decisions
• 14 Networks record treatment decisions in
patients notes from which:
• 30% - Doctors relayed treatment decisions to
patients
• 23% - Treatment decisions given at next clinic
• 46% - CNS advises patients
• With 30% of Networks use a combination of the
above three
