On adoption of regulations for medical use of the narcotic, psychotropic substances and
precursors which are subject to control in the Republic of Kazakhstan

the special prescription pink format A6 blank printed
typographically with the code, blank number and
protection level. Code includes an administrativeterritorial unit identification number of the Republic
of Kazakhstan

code, blank number and protection level.
Code includes an administrative-terrotorial unit identification number of the Republic of Kazakhstan
Special prescription blank is to be signed by the Health organization Manager or Deputy and to be
verified by the seal of health organization. Herein the seal is placed at the cut line. Moreover, the stamp
of the health organization is set, the medical documentation number, prescribing physician’s full name,
and patient’s full name and home address are indicated.
