corner.keep on file 2 years.

•sch II invoices attached with Form 222
•sche II invoices are filed away from sch 3 4 5
•exempt narcotic log---name and address of buyer, drug, date, price--RpH must see
transaction.
•master formula sheet---compounding paperwork
•MSDS
•MAR--medication administration record
•nonsterile compounded records
•poison log--name address, drug, use, buyer signature
•rx hard copy ( has initials of whoever filled rx)
•repacked meds
