•must have doc name, address phone, DEA
number, date, pt name, pt address
•drug name, strength, dosage form, quantity,
instructions(sig), refill info
•dr signature
•maybe --no substitution allowed, dispense as
written, NPI number
•e-prescribing---must have software linked. But if
drug is sch II must have original paper before
giving to pt unless emergency.
drug order
• Medicare DME---walkers, crutches,
wheelchairs, supplies, ect
• rx needs pt name, address, dr name address,
ICD code, product name, direction for use, date
and quantity
• DMV rx??---animal name and species, owner
name address
hospital drug order
– mostly electronic. goes to pharmacy for DUR. if
approved then released into computer and nurse
will give the meds.

Emergency order for sch2 fax or electronic------Only enough to cover period of emergency
Must be transferred to writing immediately
RPh must make effort to verify the order came from
authorized prescriber
Prescriber must give written rx to pharm in 7 days.
Must say ‘authorization for emergency dispensing’.
Must notify DEA if rx is no-show.
