member cost sharing effective January 1, 2013

Testing for human papillomavirus (HPV)
Counseling for sexually transmitted infections
Screening and counseling for human immunodeficiency virus (HIV)
FDA-approved contraception methods and contraceptive counseling
Breastfeeding support, supplies and counseling
Screening and counseling for interpersonal and domestic violence
Coverage for Generic Birth control at no cost to member

in-network labs are covered at 100%, no deductible, for both diagnostic and
preventive covered services
 Hospital labs are typically apply deductible/coinsurance
◦ Primary Care Physician: family practice, general practice, internist, pediatrics,
OB/GYN, or Geriatric, and behavioral health providers
All other providers classified as specialists
